Fuel alarms occurred: - 1/3/2022 on L7(diesel STP sump) REGULATION GUIDANCE: During the inspection, the designated operator shall visually inspect all spill containers, fill pipes,fill caps, and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit,for damage, and for the presence of leaks,water, or debris, and check that the sensors are located properly. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are inspecting all of the required areas and noting it on the on the reports. 2)OBSERVATION: Designated operator reports were not signed within the required time frame. -The 8/19/2021 designated operator report was signed by the owner/operator on 8/30/2021. -The 5/19/2021 designated operator report was signed by the owner/operator on 5/24/2021 REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class ll. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 2/10/202 and 2/20/2022 were completed on the same form and did not list employee initial training dates in the"Initial Training Date"section. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and on separate forms for each training date. Submit a copy of the updated forms to the EHD.
