S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> JD Service Station, Inc. 9015 WALNUT GROVE RD, THORNTON April 26, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -No alarm history was observed for the 7/26/2021 report <br /> -No alarm history was observed for the 8/26/2021 report <br /> -The 9/24/2021 report was missing the alarm history for the 91 fill (L11), 87 annular(1-12), 87 Fill sump(L13), and <br /> 87 STP sump(1-14)alarms <br /> -The 11/26/2021 report was missing the alarm history for the L 14 alarm <br /> -The 1/26/2022 report was missing the alarm history for the 91 STP(1-10), L11, L12, L13, and L14 alarms <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> ----------------------------------------------------------- <br /> OBSERVATION: The designated operator failed to check and note the correct test dates for the secondary <br /> containment and overfill testing on the designated operator inspection reports. The correct date for the secondary <br /> containment testing is 2/25/2020. The correct testing date for the overfill testing is 6/29/2021. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 26, 2022. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
