The following is an itemized list of hazardous materials violations that have not been <br /> addressed for United #5446 as of August 01, 2022. <br /> Open violations from March 29, 2022 inspection <br /> Violation#5-Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site. <br /> OBSERVATION 223.398 pounds of propane were found on site. Only 1782.5 cubic feet(48.84 gallons) of propane <br /> was reported in CERS. Propane should be reported in gallons in CERS. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6 -Failed to electronically submit a site map with all required content. <br /> OBSERVATION The site map is incomplete. The following elements are incomplete or missing: <br /> -Propane was observed on the east side of the store. The site map has propane on the north and south sides of the <br /> store. <br /> -The emergency shut off for the fuel tanks is not included on the site map. <br /> -Internal roads are not included on the site map. <br /> -Identify the contents of the underground tanks on the site map <br /> REGULATION GUIDANCE:Asite map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 9 <br />
