The following is an itemized list of hazardous materials violations that have not been addressed for BUFFALO WILD WINGS as of July 21, 2022. Open violations from May 11, 2022 inspection Violation#4-Failed to electronically submit the Business Activities or Owner/Operator Identification page. OBSERVATION: The Business Owner/Operator Identification form was not complete.The primary emergency contact, secondary emergency contact, and the environmental contact have not worked for the business in three years. Update the business owner and operator identification page. REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#5-Failed to electronically submit hazardous materials inventory for all reportable materials on site. OBSERVATION: The Hazardous Materials Inventory forms for carbon dioxide were not complete. Obsolete federal hazard categories was submitted on the chemical fact sheet. Un-mark the obsolete federal hazard categories. Use an updated Safety Data Sheet(SDS)to add the new federal hazard categories. REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part of the business plan. OBSERVATION: The site map is incomplete. The site map is missing emergency response equipment(fire extinguishers,first aid kit), an evacuation staging area, and storm drain locations. REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
