All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the 91 relay.A"Failure to Obtain a Permit" <br /> penalty applies. Ensure that the EHD is notified 48 hours in advance before testing is conducted. <br /> -------------------------------------------------------- <br /> OBSERVATION: There was a change in ownership in 2021. The first CERS permit application was dated <br /> 3/27/2022.All required elements were not submitted in CERS within 30 days of the change in ownership. <br /> REGULATION GUIDANCE: Permits may be transferred to new underground storage tank owners if: <br /> 1)the new UST owner does not change conditions of the permit; and <br /> 2)the transfer is reported to the local agency by submitting all of the required submittal elements in CERS within 30 <br /> days of <br /> the change in ownership <br /> CORRECTIVE ACTION: Immediately log into CERS and submit all required elements. <br /> This is a Class II violation. <br /> 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest <br /> opportunity. <br /> OBSERVATION: Test boots on the piping were not pulled back. Schrader valves were open, but pointed up and not <br /> positioned in a way that leaks will be detected at the earliest opportunity. <br /> REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to ensure that any <br /> releases in the piping will be detected at the earliest opportunity. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 7/15/2022. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
