Order <br />2009-0009-DWQ amended by 2010-0014-DWQ & 2012-0006-DWQ <br /> 33 <br />Effective two years after the adoption date of this General Permit, a <br />QSD shall have attended a State Water Board-sponsored or approved <br />QSD training course. <br /> <br />2. The discharger shall list the name and telephone number of the <br />currently designated Qualified SWPPP Developer(s) in the SWPPP. <br /> <br />3. Qualified SWPPP Practitioner: The discharger shall ensure that all <br />BMPs required by this General Permit are implemented by a Qualified <br />SWPPP Practitioner (QSP). A QSP is a person responsible for non- <br />storm water and storm water visual observations, sampling and <br />analysis. Effective two years from the date of adoption of this General <br />Permit, a QSP shall be either a QSD or have one of the following <br />certifications: <br /> <br />a. A certified erosion, sediment and storm water inspector registered <br />through Enviro Cert International, Inc.; or <br /> <br />b. A certified inspector of sediment and erosion control registered <br />through Certified Inspector of Sediment and Erosion Control, Inc. <br /> <br />Effective two years after the adoption date of this General Permit, a <br />QSP shall have attended a State Water Board-sponsored or approved <br />QSP training course. <br /> <br />4. The LRP shall list in the SWPPP, the name of any Approved Signatory, <br />and provide a copy of the written agreement or other mechanism that <br />provides this authority from the LRP in the SWPPP. <br /> <br />5. The discharger shall include, in the SWPPP, a list of names of all <br />contractors, subcontractors, and individuals who will be directed by the <br />Qualified SWPPP Practitioner. This list shall include telephone <br />numbers and work addresses. Specific areas of responsibility of each <br />subcontractor and emergency contact numbers shall also be included. <br /> <br />6. The discharger shall ensure that the SWPPP and each amendment will <br />be signed by the Qualified SWPPP Developer. The discharger shall <br />include a listing of the date of initial preparation and the date of each <br />amendment in the SWPPP. <br /> <br />VIII. RISK DETERMINATION <br /> <br />The discharger shall calculate the site's sediment risk and receiving water risk <br />during periods of soil exposure (i.e. grading and site stabilization) and use the <br />calculated risks to determine a Risk Level(s) using the methodology in
