<br />Site Approval Application No. PA -1900240 and Development Agreement Application No. PA -2000019
<br />was approved by the Board of Supervisors on November 8, 2022. The effective date of approval is
<br />November 8, 2022. This approval will expire for Phase 1 on November 8, 2027, which is five (5) years
<br />from the effective date of approval, and will expire for Phase 2 on November 8, 2032, which is ten (10)
<br />years from the effective date of approval, unless (1) all Conditions of Approval have been complied
<br />with, (2) all necessary building permits have been issued and remain in force, and (3) all necessary
<br />permits from other agencies have been issued and remain in force.
<br />Unless otherwise specified, all Conditions of Approval and ordinance requirements shall be fulfilled
<br />prior to the establishment of the use and the issuance of any building permits. Those Conditions
<br />followed by a Section Number have been identified as ordinance requirements pertinent to this
<br />application. Ordinance requirements cannot be modified, and other ordinance requirements may
<br />apply.
<br />COMMUNITY DEVELOPMENT DEPARTMENT (Contact: [209] 468-3121)
<br />a. BUILDING PERMIT: Submit an "APPLICATION -COMMERCIAL BUILDING PERMIT". The Site Plan
<br />required as a part of the building permit must be prepared by a registered civil engineer or licensed
<br />architect. This Plan must show drainage, driveway access details including gates, on-site parking,
<br />landscaping, signs, existing and proposed utility services, and grading (refer to the "SITE PLAN
<br />CHECK LIST" for details). Foundation and soils investigation shall be conducted in conformance with
<br />Chapter 18 of the California Building Code at the time of permit application. A fee is required for the
<br />Site Plan review. (Development Title Section 9-884)
<br />b. APPROVED USE: This approval is to establish a hospital campus with accessory medical office
<br />building facility in 2 phases over 10 years, as shown on the site plan dated January 9, 2020. (Use
<br />Type: Public Services — Essential)
<br />Phase 1, with permits to be issued within 5 years, to include the construction of a 36,000 -square -foot,
<br />single -story, 12 -bed hospital including alternative birthing services (including labor and delivery),
<br />emergency room, emergent medicine, and outpatient surgery services.
<br />Phase 2, with permits to be issued within 10 years, to include the construction of an additional
<br />140,000 -square -foot, three-story, 100 -bed, full-service hospital with emergency helistop; and a
<br />related 60,000 -square -foot, two-story, medical office building.
<br />c. CAPITAL FACILITY FEE: This project may be subject to the Capital Facility Fee. If the Capital Facility
<br />Fee is applicable, the County shall collect the fees before the issuance of any building permits.

<br />d. MITIGATION MONITORING AND REPORTING PROGRAM: The project shall comply with all
<br />required mitigations included in the MMRP dated August 2022.
<br />e. LOT ADJUSTMENT/MERGER: A Lot Line Adjustment or Merger must be approved, finalized, and
<br />perfected prior to the issuance of any building or grading permits for Phase 1. A fee is required for
<br />these applications.
<br />f. AGRICULTURAL MITIGATION: Agricultural mitigation consistent with Agricultural Mitigation
<br />Ordinance in effect at the time must be provided prior to issuance of any grading or building permit, or
<br />at an earlier date should the applicant elect. (Development Agreement PA -2000019)
