Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00054346 Site Location: 11772 E HARNEY LN <br />Activity Summary <br />Activity Date Recorded by <br />11/18/2021 Site visit, met My. Olguin onsite 707-953-3302. Viewed a septic tank (one circular GIRARDI <br />tank approximately 4 feet wide) located 16 feet south of the existing residence. <br />There are no records that show this tank (house was built in 1906). This tank has <br />an influent line from the house and an effluent line. Probed the area next to the <br />tank at the effluent line and did not locate any additional tanks. It appears that the <br />effluent line is plumbed to a second circular tank approximately 23 east of the <br />house which is filled with broken concrete, opened and exposed. No septic <br />destruction permits on file. Therefore, the existing septic tank considered as <br />partially destroyed without permits and shall be fully destroyed and replaced under <br />proper permits. Issued NAI and provided copy to My Olguin. <br />12/07/2021 Received no response from Jose Olguin since last site visit. Contacted Jose GIRARDI <br />Olguin 707-953-3302, he indicated that he found a separate (first) tank in front of <br />the house next to the circular tank that was previously pumped. The plumbing is <br />repaired. However, no progress is made on the second tank to the east that <br />appears to be destroyed. He had to end the phone call and will contact me later. <br />Charge time. <br />GIRARDI 12/08/2021 Received no returned phonecall from Jose Olguin. Phoned Mr. Olguin <br />707-953-3302. Mr. Olguin indicated that his plumber found that all of the waste <br />water lines inside the house are directed to the first compartment of the septic <br />tank that was found in front (south) of the house. There are no EHD records for <br />this tank. Therefore, they will need to identify the locations of the existing lines <br />coming out of the tank to determine if leachlines exist (or a tightline directed to the <br />tank to the east of the house, (by digging small trenches at selected locations). <br />They also need to verify the location of the septic system to the east that is shown <br />on the EHD records. I told Mr. Olguin that the configuration of the septic system <br />(s) will need to be identified and schedule a follow-up visit by 12/17/21. A <br />professional contractor is encouraged but not required. Charge time. <br />12/15/2021 Follow-up phone call to Jose Olguin; He is having the first tank pumped out <br />tomorrow and to have the configuration of the septic system evaluated he will <br />contact me when completed tentatively this Friday. I told him that there is a good <br />possibility that the configuration may include the two tanks, a leachline, and a pit, <br />but confirmation is needed. Charge time <br />01/20/2022 Contacted Jose Olguin and discussed the current status. He will send receipts of <br />the 1st tank being pumped out. He is proposing to replace the (e) Leachline and <br />connect to the (e) pit. I told him that he will need to submit a repair or replacement <br />permit. A standard repair is a 55' leachline and a 36" pit in the clay area. <br />Received receipts of pumped tank. He indicated that he will find a contractor and <br />have them contact me. Charge time. <br />01/26/2022 Contacted By Jose Ogulin Jr (Jose's Son). Told him that the they need to hire a <br />septic contractor to find the location of the leachlines and evaluate the condition of <br />the system to determine if a repair or replacement permit is needed.
