DISTRIBUTION SYSTEM INVESTIGATION CORRECTIVE ACTIONS : <br /> Make required corrections to the system to eliminate the entrance of bacteria into the system . Flush system and all dead end lines with clean well water. Disinfect <br /> the system with NSF 61 approved chlorine at 100 ppm for a contact period of at least 3, but not more than 24 hours at 10 ppm , perAWWA standards. Ensure <br /> every hose bib, faucet, toilet, shower, and any dead end line is flushed with a 100 ppm residual , by testing each site using a field test kit or for a chlorine odor <br /> where testing is not practical. Test for a 100 ppm chlorine residual, after two hours through out the system. If the residual tests below 100 ppm , increase the <br /> concentration to back 100 ppm. A successful disinfection requires a residual of 100 ppm to be present after the 3 hours, or at least 10 ppm after 24 hour contact <br /> period . If the residual is found to be non-detectable (ND) at a location , with 50-10 ppm at other locations, or with ND residuals through out the system , then the <br /> disinfection process may not have been successful. This may be an indication of where bacteria are entering into the system . Repeat the investigation , flushing , <br /> and disinfection processes. After the disinfection process, flush the system until it tests non-detect for chlorine residuals. Resample by directing your laboratory <br /> to collect " Other" samples and test for chlorine residuals and TC bacteria, using enumeration test methods from locations that were effected. NOTE : The month <br /> following a TCR MCL, you are to direct your laboratory to collect 5 ROUTINE samples and have them test for chlorine residuals and TC bacteria , using <br /> enumeration test method . If the DISTRIBUTION SYSTEM Continues to Fails, contact the EHD for guidance at (209) 468-3420. <br /> SUMMARY: Provide a report of the investigation that includes the cause of the TCR MCL, the physical repairs that were made to the system to eliminate the <br /> entrance of bacteria into the system , and the disinfection process that eliminated the presence of bacteria, and returned the system to compliance with the TCR: <br /> INVESTIGATION FINDINGS: 'r <br /> ffff.: <br /> I' <br /> REPAIRS MADE, OR TO BE MADE, TO EXLUDE THE ENTRANCE OF BACTERIA AND DATE OF THE CORRECTION : ;. <br /> i <br /> DISINFECTION PROCESS : <br /> @i@ <br /> C'. <br /> ti <br /> Submit this completed form to the EHD , by fax at (209) 468-0341 , or Mail to SJC-EHD, SPWS Program , 1816 Hazelton Ave, Stockton Ca, 95205, within 30 days of the system failing a <br /> the TCR, along with additional TC results that indicate the system is now incompliance with the TCR. Failure to submit form will result in a Procedural Violation fee of $250. <br /> i <br />
