(a). or (b) of this j <br />provision shall make the following certification: <br />"i certify under penalty of law that -this document arid all <br />attachments were prepared under my direction or <br />supervision in accordance with a system designed to <br />assure -that qualified -personnel properly gather.and <br />evaluate the information .submitted. Based on my inquiry . <br />Of the person or persons who manage the -system or <br />those. persons directly responsible for gathering the <br />information, the information submitted is,.to the best of <br />my knowledge and. belief, true, accurate, and complete. I <br />am aware that there are significant. penalties for <br />submitting false information, including the possibility of . <br />fine and imprisonment for knowing violations." <br />[CWC 13263, 13267, and 13268] <br />3. Monitoring Repoes <br />a. Monitoring results shall be reported at the intervals specified in the <br />permit. <br />b. Mbnitoring results must be reported on a Discharge Monitoring <br />Report (DMR) form oi• forms approved by the 'RWQCB or SVVRCB for <br />reporting results of monitoring of pollutants and sludge use or <br />.disposal practices. <br />c. Calculations for all limitations which -require averaging Of <br />measurements shall. utilize an arithmetic, mean unless otherwise <br />specified in this permit. <br />4. Elanned Change <br />The discharger shall tale with the appropriate RWQCB a report of waste <br />discharge at least 120 days before making any material change or proposed <br />change in the character, location or volume of the discharge. <br />
