IaiSTRIBUTION SYSTEM INVESTIGATION CORRECTIVE ACTIONS:
Wake required corrections to the system to eliminate the entrance of bacteria into the system. Flush system and all dead end lines with clean wail water. Disinfect
the system with NSF 61 approved chlorine at 100 ppm for a contact period of at least 3, but not more than 24 hours at 10 ppm, per A"A standards. Ensure
every hose bib, faucet, toilet, shower, and any dead and line is flushed with a 100 pprn residual,by testing each site using a field test kit or for a chlorine odor
where testing is not practical. Test for a 100 ppm chlorine residual, after two hours through oul the system. If the residual tests below 100 ppm, increase ttte
concentration to back. 100 ppm, A successful disinfection requires a residual of 100 opm to be present after the 3 hours, or at least 10 parr► after 24 hour contact
period. If the residual is found to be non-detectable (ND)at a location, with 50-10 ppm at other locations, or with NO residuals through out the system, then the
disinfection process may not have been successful. This may bg an indication of where bacterig arq entering into the system. Repeat the investigation, flushing,
and disinfection processes. After the disinfection process, flush the system until 9 tests non-detect for chlorite residuals. Resample by directing your iabor-atoay
to collect"Other"samples and test for chtoriruresiduals and TC bacteria, using enameratign 11gi mefttods from locations that were effected. NOTE: The nwntfi
following a TCR NCL, you are to direct your laboratory to collect 5 ROUTINE samples and have them test for 01ort to residuals and TC bacterue, usin
enumeration test method. tt_the QE5TRIBUTIC3N SYSTEM Continues to Fails, contact the EHD for guidance at(209)466-3420.
,d+,ria a ripkrt Of ffte i
INVESTtGATiON FINDINGS:
-11 CA6ndT 1L C c.,l eel CoQ 5
OQ 1P
REPAIRS MADE, OR TO BE MADE, TO EXLUDE THE ENTRANCE OF BACTERIA AND DATE OF THE CORRECTION.
t
DISINFECTION PROCESS: 4-05 4 IC-e✓1
V n,,A I vi A-U z0 car-A oserA w o40
57t a
e,1L -t ln-L C_ w e 'uS
sQ cn je, o fnV= Z4 H 0
-r O S E W\260- tV\L C-L2 I<-Q C t w Oi S
5 t u l'v ,
Submit this completed form to the EHO,by fax at(209)468-0341,or Mail to SJC-EHD, SPWS Program.600 l=ast gain Street,Stockton Ca,95202,within 30 days of the system
failing the TGR,along with additional TC results that indicate the system is now tacomplianoe with the TCR.
