Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COYLIPOTE Site Location: 21908 S MANTECA RD <br />Activity Summary <br />Activity Date Recorded by <br />03/15/2022 Phoned Jason Moody of Atlas Environment to verify if the clean up will proceed. SAETERN <br />Per Jason Moody, he will be there a little after 9 AM. <br />Arrived on site with Robert McCellon, Program Coordinator. Met with Jason Moody <br />and his crew. The gates was locked and a key was not obtained. The assistant for <br />Mr.Jagroop Singh had to obtain a bolt cutter to let us in after she had discussed it <br />with Mr. Jagroop. <br />Observed white powdery substance and stain on the soil. Per Jason Moody, he <br />had pH the powder and it was "a little under a 7". He believed the white powder <br />substance is soap. Stained soil and white powder (soap) was placed into a 55 <br />gallon plastic blue drum and the rest of the stained soil was placed into a 55 gallon <br />black plastic drum. <br />A non-RCRA hazardous waste manifest was documented (see photo) and both <br />drums was labeled as hazardous waste. <br />Observed backfilling for the previous three holes that was part of the clean up. <br />03/22/2022 Generated photolog. SAETERN <br />04/11/2022 Generated photo log for 3-15-2022 SAETERN <br />04/11/2022 Emailed Jason Moody of Atlas Environmental for copies of the final manifests VANG-LEE <br />016468277FLE and 016996354FLE. I was able to locate a final copy of manifest <br />016468276FLE in the department of toxic substances control's hazardous waste <br />tracking system. <br />04/22/2022 Attempted to obtain final signed copies of manifests 016468277FLE and <br /> <br />VANG-LEE <br />016996354FLE through the Department of Toxic Substances Control's website but <br />was unable to locate the final copies. Emailed jason@atlasenvironmental.net and <br />to4152615400@gmail.com requesting for the missing manifest. <br />05/02/2022 Completed prop 65. I received final copies of manifests 016996354FLE and VANG-LEE <br />016468277FLE via email on 04/25/2022 from nicole@atlasenvironmental.net. <br />05/04/2022 Printed the complaint investigation form (5104), complaint investigation summary VANG-LEE <br />(5105), and daily time and activity summary (Log2). Prepared complaint file and <br />abated complaint. Submitted complaint file for final review. <br />5105.rpt <br /> Page 3 of 3
