Mountain House Pkwy. <br /> Good Afternoon, <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of the underground storage <br /> tank installation application for 19533 S. Mountain House Pkwy., Mountain House (SR0083832). Please provide the <br /> following additional information and address the following comments: <br /> 1. Page 3 of the Application for Underground Storage Tank(UST) Installation Permit(application): Contractor <br /> information and Fire Permit information were not included. <br /> 2. Page 3 of the application:the certification statement regarding worker's compensation laws of California was <br /> not signed by the owner or contractor. <br /> 3. Page 6 of the application:the tank capacity for each product does not match the information in the plans. <br /> 4. Page 7 of the application and the overfill prevention device cut sheet indicate the overfill prevention device is <br /> OPW model 7150,which is non-testable.The plans state the overfill prevention device is OPW model 71SO- <br /> 410CT,which is testable. Clarify which overfill prevention device model will be used and provide an updated <br /> application, parts list, plans and/or cut sheets. <br /> 5. Page 8 of the application:the alarm panel model listed is for a sensor. Update the application with the correct <br /> alarm panel model. <br /> 6. Plans page FT-1 indicate the burial depth is 5-7 feet. Page FT-1, Item#12 of the UST Anchoring Notes, states <br /> tanks will be installed so that the top of the tanks are at the same height.The buoyancy calculations state the <br /> tank burial depths are different. Update the plans and/or buoyancy calculations to indicate the accurate tank <br /> burial depth information. <br /> 7. Plans page FT-1, Item#13 of the UST Installation Notes, states the sump monitoring sensor is to rest on the tank <br /> rib. Drawings on Pages FT-2—3 show the submersible turbine pump (STP) and fill sump sensors resting on the <br /> tank ribs. Update the plans so that the sensors are installed/maintained/located to detect leaks at the earliest <br /> opportunity. <br /> 8. Plans page FT-7 shows two product pipes connected to each of the three STP sumps. Plans page FT-2 shows one <br /> product pipe connected to each STP sump. Update page FT-7, FT-2, and/or the parts list to indicate the accurate <br /> number of product pipes connect to each STP sump. <br /> 9. Plans state all sumps are brine-filled.The vacuum zones on plans page FT-7 include STP,fill,vent, and dispenser <br /> sumps. Page FT-7 does not show any vacuum lines to the sumps. Clarify which components are brine-vs. <br /> vacuum-monitored, and update the vacuum zones on page FT-7 of the plans. <br /> 10. Plans do not indicate brine test ports in the STP,fill, and vent sumps. Provide an indication of brine test ports in <br /> the STP, fill, and vent sumps. <br /> 11. Plans do not indicate that the spill containers have shroud boots and clamps. Provide an indication that the spill <br /> containers have shroud boots and clamps. <br /> 12. Plans indicate ball valves will be placed on vacuum tubing. No additional shut off valves should be installed <br /> between the monitored interstitial spaces and their respective sensors. <br /> 13. Plans do not indicate longest piping run. Provide an indication of the longest piping run. <br /> 14. A cut sheet for Veeder-Root 304 sensor was included.The Veeder-Root 304 sensor was not included in the parts <br /> list or scope of work. Clarify whether Veeder-Root 304 sensor will be used and provide an updated application, <br /> parts list, and/or cut sheets. <br /> 15. The parts list states the dispenser model is Bravo model B9250.The dispenser cut sheet provided is for Bravo <br /> model B9000. Clarify which dispenser model will be used and provide an updated application, parts list, and/or <br /> cut sheets. <br /> 13 <br />
