Per instructions the following violations have been corrected. Please let me know if
this will suffice in bringing the store back into compliance or if you have further questions.
CCR 66262.11 Failed to determine if a waste is a hazardous waste.
OBSERVATIONS: One container was observed on-site of RAINX WTR REP ARS and was labeled
"DAMAGED",
"MANAGER DISPOSE", Rtn date: 1-17-22.
The product was placed in the blue bucket and waiting for service pick-up.
CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years.
OBSERVATION: Hazardous waste generator signed copy of Uniform Hazardous Waste Manifest (UHWM)
number 200160534 CLE, shipment date 5-23-22, was not found on-site.
During a training session, Safety-Kleen trainee created manifest# 007440555SKS w/o the trainers knowledge.
At the same time the actual waste expert for SK created#007476628 SKS for the same waste. Manifest that
the trainee created was never sent to the DISC, and was stapled to the actual manifest. So there was disposal
copy of the manifest.
CCR 66266.81(a)(4)(B) Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3
years.
OBSERVATION: Complete bills of lading for the recycling of lead acid batteries for the 2022 calendar year
were
not found on site
This violation is in regards to the BOLS of 2022 being cut off from printing, however there is no
possible way to redo these manifests without falsifying and changing the dates. I have made our IT
aware that the printer sometimes cuts off part of the Bill of Lading. (See attached photo)
CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days (90 days acute / extremely).
OBSERVATION: The following hazardous waste containers were found on-site with an accumulation start date
of
greater than 180 days or no accumulation start date:
The items that were noted on the shelf have been properly disposed of in the red bucket as required.
I
