S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> AUTOZONE#6897 1 10336 TRINITY PKWY, STOCKTON June 20, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: Some designated facility signed and dated copies of Uniform Hazardous Waste Manifests <br /> (UHWM)were not found on-site. <br /> The missing manifests are: 200108898CLE, shipment date 3-18-22 and 200108899CLE, shipment date 3-18-22. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in <br /> accordance with section 66262.23(a)for three years or until the generator receives a signed copy from the <br /> designated facility which received the waste. This signed copy shall be retained as a record for at least three years <br /> from the date the waste was accepted by the initial transporter. The hazardous waste generator must submit a <br /> copy of the manifest to the California Department of Toxic Substances Control (DTSC)within 60 days if the <br /> destination facility signed manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate the destination facility signed manifests listed above and maintain in the facility <br /> file for three years from the date of shipping. Send a copy to the San Joaquin County Environmental Health <br /> Department(EHD). <br /> This is a Class II violation. <br /> 112 CCR 66268.7(a)Failed to make determination for land disposal. <br /> OBSERVATION: Land Disposal Restriction Notification forms were found on-site without certification signatures by <br /> the hazardous waste generator. The unsigned forms were associated with these uniform hazardous waste <br /> manifests: 015096638FLE, 016246049FLE, 016246048FLE and 016246050FLE. <br /> REGULATION GUIDANCE: The generator shall determine if the waste is restricted from land disposal prior to <br /> transport off-site and keep complete documentation on-site for three years. See Title 22 California Code of <br /> Regulations section 66268.7. <br /> CORRECTIVE ACTION: Provide a corrective action statement and supporting documentation within 30 days to the <br /> San Joaquin County Environmental Health Department. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 20, 2022. <br /> Wastes generated by this facility: aerosols, corrosive solids, flammables, universal waste- batteries, lead acid <br /> vehicle batteries, waste alcohols, oily debris.
