The following is an itemized list of hazardous materials violations that have not been addressed for APPLEBEES as of August31, 2022. Open violations from July 13, 2022 inspection Violation #6 - Failed to electronically submit a site map with all required content. OBSERVATION The site map is not complete. The site map does not show/address the following required elements: 1 . North orientation identifies the direction of south as north. 2. Loading areas. 3. Internal roads. 4. Storm drains. 5. Access and exit points from the adjacent streets. 6. Emergency response equipment (fire extinguishers). 7. Codes/acronyms used on the map are not defined. REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa,ca,gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. OBSERVATION Documentation showing relevant initial and annual refresher training was not available. REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment (C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
