The following is an itemized list of hazardous materials violations that have not been <br /> addressed for A&W Root Beer of Lodi as of September21, 2022. <br /> Open violations from August 02, 2022 inspection <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> OBSERVATION The site map is not complete. The site map does not show/address the following required <br /> elements: <br /> 1 . Internal roads. <br /> 2. Storm drains. <br /> 3. Access and exit points from the street. <br /> REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 - Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION The emergency response(ER) procedures were lacking or inadequate. 1 . The location of <br /> the Emergency Response Equipment has not been identified. 4. The Earthquake Vulnerability section has not been <br /> completed. <br /> REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> OBSERVATION The Emergency Response/Contingency Plan has not been annually certified/submitted as a part <br /> of the business plan . <br /> Page 1 of 2 <br />
