The site map does not show/address the following required <br /> elements: <br /> 1. North orientation. <br /> 2. Loading areas. <br /> 3. Internal roads. <br /> 4. Adjacent streets. <br /> 5. Storm drains. <br /> 6. Access and exit points for structures and from the street. <br /> 7. Emergency response equipment (fire extinguishers). <br /> 8. Emergency shutoffs. <br /> REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. If a site map element is not applicable <br /> for your facility then list it on the map and label as "NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class 11. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER) procedures were lacking or inadequate. A blank Consolidated <br /> Emergency Response/Contingency Plan was submitted. The submittal did not include: 1. Procedures for immediate <br /> notification of local emergency personnel and the Environmental Health Department (EHD). 2. Procedures for the <br /> mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the <br /> environment. 3.
