SAN ,dDAQ U I N Environmental Health Department <br /> C01-1 N T <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> O'REILLY AUTO PARTS#2917 3228 E HAMMER LN, STOCKTON July 27, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 117 CCR 66265.16 Failed to maintain complete hazardous waste training records. <br /> OBSERVATION: The business operator could not produce documentation during the inspection that demonstrates <br /> employees have complete hazardous waste training as specified in this section. <br /> REGULATION GUIDANCE: <br /> New employees shall be trained within 180 days of hire and annually thereafter as specified in Title 22 California <br /> Code of Regulations section 66265.16.The owner or operator shall maintain training records on current personnel <br /> until closure of the facility and training records on former employees for at least three years from the date the <br /> employee last worked at the facility. <br /> CORRECTIVE ACTION: Immediately train all employees who work with hazardous waste in compliance with the <br /> time frames and training materials required by this section. Provide training documentation to the San Joaquin <br /> County Environmental Health Department(EHD)within 30 days. The documentation must include a list of training <br /> topics, date of training,job title, name of employee,written job description (qualifications and duties), and employee <br /> signed (or certified)record that documents the training. <br /> This is a repeat violation, Class II. <br /> 118 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: The facility could not provide all of the consolidated manifest receipts for hazardous waste <br /> transported from the site in the past three years. The missing receipts include: <br /> State waste 223, 250 pounds, shipment date 1-29-21. <br /> State waste 221 <br /> 12/20/2019, 220 gallons used oil, shipment date 12/20/2019 <br /> 5/3/2021, 220 gallons used oil, shipment date 5/3/2021 <br /> 9/10/2021,200 gallons used oil, shipment date 9/10/2021 <br /> 9/17/2021, 100 gallons used oil, shipment date 9/17/2021 <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action <br /> regarding the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Locate a copy of the receipts listed above.
