The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Sunwest Chevron as of August 16 , 2022 . <br /> Open violations from October 13 , 2021 inspection <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> OBSERVATION A site map was not completed and submitted as part of the business plan . The site map is <br /> incomplete . <br /> - Fire extinguishers were found between the dispensers and emergency shutoff and were not listed on the map. <br /> -The storm and sewer drains located near/by the dispensers were not listed on the map . <br /> - Internal roads were not listed on the map. <br /> -The propane is not listed on the map . <br /> REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas , internal roads , adjacent <br /> streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous <br /> material handling and storage areas , and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as " NA" . <br /> CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System ( CERS ) at <br /> http : //cers , calepa . ca , gov/, upload the correct or updated information , and submit to the EHD for approval . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #9 - Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS ) annually by the due date , January 15 . <br /> -The 2019 HMBP was submitted on time on 12/ 11 /2018 but was incomplete . <br /> -The 2020 HMBP was not submitted . <br /> -The 2021 HMBP was submitted on time on11 /9/2020 but was incomplete . <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year . <br /> CORRECTIVE ACTION : Immediately log into the CERS at http : //cers , calepa , ca . gov/ , enter the correct or updated <br /> information , and submit to the EHD for approval . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Page 1 of 2 <br />
