Observed dead cockroach in washing <br />solution in 2 comp sink and dead cockroach on right drainboard. Observed live cockroach on wall near buffet tables and few <br />dead on the floor near the buffet tables. Observed live cockroaches on wall and floor in kitchen, on wall and floor near <br />dishwasher, and on floor in pantry room. Observed live cockroach on right prep sink drainboard that fell into sink, observed <br />some dead cockroaches on right prep sink. Observed few dead and one live cockroach on dishwasher drainboard. Clean <br />and sanitize areas immediately and increase pest control measures. Keep facility closed, call for re-inspection prior to <br />re-opening. <br />Observed dead gnat on top of plastic wrapped fish in sushi bar Everest display. Observed live gnats in kitchen near <br />dishwasher. Replace plastic wrap and increase pest control measures. <br />Observed live flies in kitchen and in dining area near buffet tables. Increase pest control measures to keep facility vermin <br />free. <br />CALCODE DESCRIPTION: Each food facility shall be kept free of vermin: rodents (rats, mice), cockroaches, flies.( 114259.1, 114259.4, <br />114259.5) <br /> #1 Demonstration of Knowledge <br />OBSERVATIONS: Food handler cards for employees not available during the inspection. Provide food handler cards for <br />employees within 30 days of hiring and maintain records on site. <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br /> #6 Handwashing Facilities Supplied and Accessible <br />OBSERVATIONS: There was no soap and paper towels in dispensers at sushi bar hand sink and at right prep sink. There <br />was no paper towels in dispenser at hand sink near soda machine. Provide soap and paper towels in dispensers today. <br />CALCODE DESCRIPTION: Handwashing soap and towels or drying device shall be provided in dispensers dispensers shall be <br />maintained in good repair. (113953.2) Adequate facilities shall be provided for hand washing, food preparation and the washing of <br />utensils and equipment. (113953, 113953.1, 114067(f)) <br /> #16 Shell Stock Regulations <br />OBSERVATIONS: Shellfish tags for oysters and clams were not kept. Maintain shellfish tags on site for 90 days after <br />finishing box. <br />CALCODE DESCRIPTION: Shell stock shall have complete certification tags and shall be properly stored and displayed.
