COMPLAINANT INFORMED AN EMPLOYEE AND THEY DID NOTHING. <br />***PLEASE CALL COMPLAINANT WITH FINDINGS. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM-- LOCATION --TEMP ° F --COMMENTS <br />No Temperature Data Collected <br />NOTES <br />On site at 3950 for complaint investigation regarding an alleged baby cockroach crawling on the table and window at the <br />facility. Met with assistant manager Nancy Lopez and discussed complaint. She was aware of the complainants allegation. <br />She reported employees went to check for the cockroach and found nothing. The meal was prepared again for the customers. <br />The complainant wanted his money back, but the meal was prepared again and eaten by customer and his party. When he <br />was not given his money back, he stated he would correct the situation. I searched facility and did not find any of the alleged <br />pests. I verified receipts from Area Wide and Eco Lab monthly pest control services at the facility. Facility was last serviced by <br />Eco Lab on 05-21-2022 and by Area Wide on 05-16-2022. <br />Inspection report was emailed to assistant manager. <br />To minimize person-to-person contact, the signature of the person receiving the inspection report was not captured. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
