Please complete the enclosed application and remit the appropriate fee at <br /> least 45 days prior to the planned date of occupancy of your facility. <br /> Please complete and return the enclosed "Statement of Citizenship, Alienage and Immigration <br /> Status in Support of a Request for State Public Benefits." This form need not be submitted if you <br /> have completed and submitted it in the past and the information is unchanged. <br /> If you do not intend to use the facility this year, please complete and return the enclosed <br /> "Certificate of Non-Operation." <br /> For your reference, the laws and regulations covering employee-housing facilities are found in <br /> the California Health and Safety Code, Division 13, Part 1, and in the California Code of <br /> Regulations, Title 25, 1 h,apter 1, Subchapte, 3. <br /> Please return the completed application and the applicable permit fees, or the Certification of <br /> Non-Operation, in the enclosed envelope.
