0!04936SV <br />June 18, 2001 <br />Page 2 of 3 <br />Failure on the permittee's part to comply with any provision will be cause for revocation of this permit <br />IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF <br />COMPLETION PROVIDED BY GRANTOR. <br />Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State Standard Specifications <br />Reference to Engineer in the State Standard Specifications shall include State Representative. <br />Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, and Business and Professions <br />Code, Section 8771. Permittee shall physically inspect the work site and locate survey monuments prior to work <br />commencement. Monuments shall be referenced or reset in accordance with Business and Professions Code. <br />A pre job conference with the permittee, contractor, and State representative is required prior to start of work. <br />Permittee shall contact the State representative and arrange the meeting. This meeting may be waived at the direction <br />of the State representative. <br />Notwithstanding General Provision #4, your contractor is required to apply for and obtain an encroachment permit prior <br />to start of work. <br />Permittee shall, prior to commencement of any work, provide the State representative with the name and phone <br />number of the person in responsible charge of the work to be performed under this permit. <br />Traffic control shall be done in conformance with Caltrans Manual of Traffic Controls 1996. <br />In addition to General Provision 35, "Notification of department and TMC", the permittee shall notify the <br />Department's representative AND THE TRANSPORTATION MANAGEMENT CENTER (TMC) immediately prior to <br />initiating any traffic control and immediately upon the removal of any traffic controls. The telephone number <br />for Caltrans District 10 TMC is (209) 948-7556. <br />Flashing arrow signs (Type 1) are required for lane/Shoulder closures on expressways or multilane conventional <br />highways. <br />Except when necessary, as determined by the State representative, no installation, construction equipment, or personal <br />vehicles shall operate or park within the traveled way. <br />In addition to the attached General Provisions, the following special provisions are also applicable for installation and <br />monitoring of groundwater monitoring wells: <br />1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: <br />A. On holidays or an afternoon proceeding a holiday. <br />B. In medians. <br />C. When work requires traffic control on lanes. <br />2. Permittee shall furnish, place and maintain required signs, safety equipment, traffic control, and warning devices in <br />accordance with the manual of Traffic Controls published by the Department of Transportation. <br />3. All personnel shall wear hard hats and orange vests, shirts or jackets as appropriate. <br />4. Unless specifically authorized in this permit, markings within the right of way shall be temporary. Paint used for <br />markings shall be water-soluble and other markers shall be removed upon completion of work. <br />5. Any collected data requested by Caltrans shall be furnished to Caltrans without charge. <br />6. Vehicles necessary for work may be parked in State Highway right of way off paved shoulders. Auxiliary support <br />and employee vehicles shall remain outside the right of way. <br />7. Permittee shall provide for the safe passage of pedestrians. �� <br />
