The site map does not include loading and unloading areas for the reportable hazardous materials on the HMBP <br /> which are the diesel fuel tank, lead acid batteries, LPG bottles( in front and behind the store), treated wood waste <br /> container behind the store, and hazardous waste storage(HWS)in the receiving area. <br /> 2. The site map does not include the location of the treated wood waste container behind the store outside the <br /> wood product area. <br /> 3. The HWS in the tool rental center is located in the incorrect location on the site map. A plumbed eyewash <br /> station in the tool rental center is not included on the site map. These items are both in the south part of the tool <br /> rental center near the spill kit(SK)and the first aid kit. <br /> 4.A plumbed eye wash station in the forklift battery recharging area is not included on the site map. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Facility could not provide the emergency response plan and procedures during the inspection. <br /> The emergency response plan currently in the California Environmental Reporting System (CERS),the <br /> Consolidated Emergency Response/Contingency Plan, was not available for review. <br /> REGULATION GUIDANCE: The emergency response plans and procedures,the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or <br /> the unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the complete business plan, including the <br /> emergency response plan, readily available for use and review by the store employees. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 17, 2022 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. <br /> FA0013652 PR0521070 SCO01 10/12/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
