We have attached a copy of the Report and provide the <br />following response. Each numbered paragraph below replies to a <br />corresponding paragraph of the Report. <br />1. During a: telephone conversation between Ms. Nancy <br />Lancaster of the Department of Toxic Substances Control (DISC) and <br />Ms. Carrie Cummings of Forward Inc. on July 22, 1992,-a list of the <br />seven specific itemsfalling under this alleged violation were <br />provided. A response to.%six of the proposed items has been <br />provided by Kleinfelder Associates in the attached"'letter dated <br />August 13, 1992. In.the attached letter, ,'Kleinfelder references <br />the Post Closure Plan for WMU A, Addendum #2, which was originally <br />prepared by Kleinfelderon August 8,11991. ,The pl6n was reprinted <br />with minor revisions on August, ;13,_1992, and, -we are enclosing a <br />copy for DISC review. andscomment`:` A .copy will also be sent to Wade <br />Cornwell of the Facility�,Perm`itting Unitfor review and comment. <br />eThe seventh item (no'tw`-addressed by Kleinfelder): The <br />inspector stated that at least one monitoring well was missing <br />a survey mark. <br />The monitoring well head elevation is measured on the northern <br />most point of the rim of the PVC well casing and the point is <br />marked with an ink pen. Our experience is that well survey <br />marks have sometimes faded. All wells will be marked using a <br />chisel on the northern most point of the PVC rim so that <br />fading will no longer be a problem. <br />2. During the July 22, 1992 telephone conversation mentioned <br />above, a list of two specific items falling under this alleged <br />violation were provided. <br />*The inspector stated that there was standing water in the <br />locking steel vault for MW -3A. <br />
