I believe the original
form I filled out was the correct one and attached there is the work plan and site map. All the piezo
specs are included on the application. There will be a bentonite seal above the sand pack at 20 ft
bgs. There is 10 ft slotted PVC, 20 ft blank, as indicated on the application. The borings are in the
Caltrans right-of-way. Please let me know if you need any more info. If I could get these approved by
the end of the week that would be great, then Caltrans could get them installed the following week.
Thank you,
Laura Gregg
Phone: 714-337-9726
Email: lgregg@greggdrilling.com
2726 Walnut Ave
Signal Hill, CA 90755
www.greggdrilling.com
From: ehsitemitigation [EHD] <ehsitemitigation@sjgov.org>
Sent: Wednesday, November 3, 2021 8:24 AM
To: Laura Gregg <lgregg@greggdrilling.com>
Subject: RE: Please Expedite - Caltrans Piezometer Permits
Laura:
For a “site mitigation” well permit for your piezometers, please visit our website, and look under
Forms:
https://www.sjgov.org/department/envhealth/forms
Click on Site Mitigation tab, there you’ll find all the permit forms you would need for your Phase II
Site Assessment or other Site Mitigation endeavor, as well as a guidance.
