The following is an itemized list of hazardous materials violations that have not been addressed for NATIONAL EXPRESS TRANSIT as of December 14, 2022. Open violations from November 07, 2022 inspection Violation#6-Failed to electronically submit a site map with all required content. OBSERVATION: The site map is incomplete. The site map is missing the location of the evacuation staging area location. The site map is also missing the location of the emergency response equipment(fire extinguishers and spill kit equipment).Add the missing elements to the site map and make a new submittal in CERS. REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#7-Failed to electronically submit an adequate ER procedures for a release or threatened release. OBSERVATION: The emergency response(ER)procedures were not complete. The evacuation staging area was not identified in section E3 of the emergency response and contingency plan.Add the location of the evacuation staging area to section E3 and then make a new submittal in CERS.
