Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />March 21, 2022 <br /> Facility Address: <br /> 4315 E WATERLOO RD, STOCKTON <br /> Facility Name: <br /> WATERLOO FOODMART <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for propane, car wash detergent, and car wash <br />wax-polisher were not complete. <br />-Car Wash Detergent is listed as a mixture, but no mixture components were listed. <br />-Car Wash Wax-Polisher is listed as a mixture, but has a CAS number listed. The CAS number matches the <br />number listed for isopropyl alcohol in the Mixture Components section. The Mixture Components section does not <br />include % by Weight for any components, or CAS numbers for three of the components. <br />-Propane does not have "Gas Under Pressure" marked as a hazard. <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: A site map was not completed and submitted as part of the business plan./The site map is <br />incomplete. The map submitted did not include labeled access and exit points. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. The 2020 information was <br />submitted late, on 2/19/2020. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: Ensure all future submittals are submitted within the required timeframes. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection.
