HERITAGE BAG CO. <br />July 14, 2003 <br />Mr. Carl Solomon <br />Kaiser Permanente <br />1800 Harrison Street <br />19th floor <br />Oakland, CA 94612 <br />Dear Mr. Solomon: <br />1J 002/002 <br />This letter will serve to certify that all Heritage Bag Company LLDPE and HMW -HDPE printed <br />biohazard infectious waste liners comply with the State of California Medical Waste <br />Management Act Section 117630 requirements for 165 grams dart impact strength as measured <br />using ASTM Test Method D 1709-85. <br />However, any bags used for off-site transport of biohazard infectious waste roust be packaged in <br />a bag certified to meet DOT regulation DOT E-12224..
