Foley, Registered Environmental Health Specialist <br />Environmental Health Department <br />San Joaquin County <br />304 East Weber Avenue, Third Floor <br />Stockton, CA 95202-2708 <br />Dear Ms. Foley, <br />Please accept this correspondence as the Kaiser Permanente Stockton Medical Centers <br />response to your findings and notice of violation of June 16, 2003. <br />During your routine inspection you made several findings to be addressed and corrected. <br />The following actions have been taken: <br />Violation, item number 16, concerning the biological indicator Bacillus <br />Stearothermophilis test, has now been addressed. Tests will be conducted in <br />accordance with the Medical Waste Management Act. All loads are to be checked for <br />attaining adequate temperature and time, if such conditions are not met, <br />documentation of reasoning and what was done with the load will be provided on the <br />San-I-Pac wheel. <br />2. Violation, item number 19, concerning trace chemo sharps not being segregated from <br />other trace chemo has now been corrected. Trace chemo sharps will now be kept <br />separate from trace chemo non -sharps. Trace chemo sharps will be placed in a sharps <br />container that is labeled in accordance with the Medical Waste Management Act. <br />Trace chemo non -sharps will be kept in a rigid container with a tight -fitting lid and <br />labeled in accordance with the Medical Waste Management Act as a secondary <br />container. Bulk chemo waste will be treated as Hazardous waste and manifested as <br />such. <br />3. Violation, item number 25, concerning whether the biohazard bags in use meet the <br />165 gram dart test. Attached are the manufacture specifications, which indicate the <br />bags meet this standard. Also note the bags being used have been replaced with bags <br />that have the biohazard symbol as well as the word `BIOHAZARD" written on them <br />and meet the 165 gram dart test. <br />08274-12 (REV. 6-00) <br />
