Harper, MHA, FACHE <br />Senior Vice President/Area Manager <br />Central Valley Service Area <br />RECEIVED <br />JUN 0 2 2011 <br />ENVIRONMENTAL HEALTH <br />PERMIT/SERVICES <br />The purpose of this letter is to respond to the following Violations of the Health and Safety Codes for <br />Kaiser Permanente Medical Office located at 7373 West Lane Stockton, CA: <br />HSC Section 18275 MW — MW red bags, path, chemo shall be labeled properly. - This facility is storing <br />medical waste in a non -red biohazard bag. Clear bags do not have appropriate medical waste labels. <br />a- Inspector observed a clear plastic bag inside a biohazardous waste container in the designated <br />accumulation area. <br />b- Inspector observed path waste placed in a clear plastic bag in a path waste container in the <br />pathology room. <br />HSC Section 117960 — LQG shall file MWMP with enforcement agency. - The facility has had a change <br />in their Medical Waste Management Plan (MWMP) since the last inspection. The Plan has not been <br />submitted to this Enforcement Agency (EA). <br />Enclosed with this letter is proof of the requested corrections for the above violations. <br />Kaiser Permanente is dedicated to abiding by all applicable regulatory requirements.
