Item completed <br /> 13 HSC 117975—LQG shall maintain treatment and shipping and tracking <br /> documents for 2 years <br /> Several treatment records from Daniels Sharpsmart Inc. do not have a signature <br /> under the "certificate of destruction section". <br /> Heidi Ridenour requested "Certificates of Destruction" for manifests 752412 <br /> and 751974 from Daniels Sharpsmart, Inc. on December 15th. Heidi e-mailed <br /> the "Certificates of Destruction" to Harprit S. Mattu(SJCEHD) on December <br /> 16th, 2015. Item completed <br /> 24 HSC 118275—Medical, biohazardous, sharps, trace chemo, pathology or <br /> pharmaceutical waste shall be segregated and contained pursuant to this <br /> section. The containers shall be properly labelled <br /> A pharmaceutical waste container located in the 2 North Soiled Utility Room <br /> did not have a label on top. Corrected at time of survey.
