Yosemite Avenue, Manteca <br /> Date Inspected: April 1, 2002 <br /> Inspected by: Kasey Foley, Lead Senior, Registered Environmental Health Specialist <br /> On Monday, April 1, 2002, Kasey L. Foley performed a medical waste inspection with Kaiser <br /> Support Services Clerk, Isabel Smith. <br /> The following violations were found: <br /> California Health and Safety Code (HSC) Sections 117890 and 117955: Kaiser <br /> Permanente generates over 200 pounds of medical waste per month and has for over three <br /> years and is not registered as a large quantity generator with the San Joaquin County <br /> Environmental Health Department (EHD). Register by April 8, 2002 and pay all fees due. <br /> HSC Section 117960: No medical waste management plan is on file with EHD or even onsite <br /> at the facility. Provide a current copy of a medical waste management plan pursuant to this <br /> section for the Manteca facility by April 15, 2002. <br /> HSC Section 117975: Medical waste tracking records for the past three years were not <br /> available for review. Provide all medical waste tracking records for the past three years by <br /> April 8, 2002. <br /> HSC Section 118275 (g): Pharmaceutical waste was stored in plastic containers with a label <br /> taped on that read "Chemicals for destruction" not "INCINERATION ONLY" on the lid and on <br /> the sides, so as to be visible from any lateral direction, as required by this section. <br /> HSC Section 118280 (b): Many biohazardous waste containers (more than 5) were not <br /> displaying the words "Biohazardous Waste" or the international biohazard symbol and the word <br /> "BIOHAZARD" on the lid and on the sides of all containers so as to be visible from any lateral <br /> direction. Provide proper labels for all containers storing biohazardous waste by April 8, 2002. <br /> HSC Section 118280 (e): Waste which meets the definition of biohazardous waste in <br /> subdivision (g) of Section 117635 (e.g. waste pharmaceuticals) was stored onsite for more <br /> than 90 days without obtaining written approval form the EHD. <br /> HSC Section 118310: The medical waste storage area had no warning signs on, or adjacent <br /> to, the exterior of entry doors, gates, or lids. Obtain warning signs that read "CAUTION-- <br /> 1 <br />
