Hazelton Ave., Stockton CA 95205 <br />Re: December 26, 2018 Annual Medical Waste Inspection- American Red Cross Stockton, CA <br />Dear Mr. Easter: <br />This letter is in response to the attached notice regarding your December 26, 2018 Annual <br />Medical Waste Inspection of American Red Cross 2888 W. March Lane Stockton, CA 95219. <br />The alleged safety hazard is described as: <br />12. The medical waste management plan (MWMP) Does not contain all information listed <br />in HSC 117960 <br />-MWMP shall containing all information listed in HSC 117960. The Medical Waste <br />Management Plan should be available on site and an updated copy of the plan must be <br />submitted to the EHD. The Medical Waste Management Plan submitted to the EHD <br />requires updating with the following information by 1/21/2019: <br />(e) The plan should be updated to reflect that no treatment occurs at the facility. -The MWMP <br />was updated indicating treatment is contracted. <br />(f) The name and business address of the registered hazardous waste hauler used by the <br />generator to have untreated medical waste removed for treatment, if applicable, and, if <br />applicable, the name and business address of the common carrier transporting pharmaceutical <br />waste pursuant to Section 118032.- The MWMP has been updated to include the business <br />address of the registered waste hauler. <br />(g) The name and business address of the offsite medical waste treatment facility to which the <br />medical waste is being hauled, if applicable. - A list of Stericycle medical waste treatment <br />facilities in the region does not meet this requirement — The MWMP has been updated to <br />specify designated treatment facilities. <br />(k) A statement certifying that the information provided is complete and accurate. - Statement <br />certifying that the information of MWMP is complete and accurate is stated under Section M. <br />13. The registered large medical waste generator does not maintain individual treatment <br />records and shipping and tracking documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain <br />individual treatment operation records, and shipping and tracking documents for all <br />untreated medical waste shipped offsite for treatment for 2 years. HSC 117975(a) The <br />facility only provided access to records for 20 months.
