Have photos documenting placement of signage on the storage shed <br />within 30 days. Completed and sent 6-16-11. <br />3. Section 118275 (g) — Pharm label/store <br />a. Pharmaceutical waste when placed in a container or secondary container shall be labeled <br />with the words "Incineration Only" on the lid and on the sides so as to be visible from <br />any lateral direction. <br />-� Pharmaceutical waste containers observed throughout this facility were <br />lacking required labeling "Incineration Only". <br />--► Place all required labeling on all pharmaceutical waste containers <br />including pharmaceutical sharps containers at this facility. Stericycle is <br />working on a solution to this issue. <br />_+ Increase awareness through training so all required labeling on all <br />medical waste containers specified in Chapter 9 Section 118275 and <br />118280 will be placed by next inspection (routine). <br />4. Section 118280 (a) (b) <br />a. Containerize biohazard bags a person shall do all of the following: <br />b. The bags shall be tied to prevent leakage or expulsion of contents during all future <br />storage handling or transport. <br />c. Biohazard bags shall be placed for storage, handling, or transport in a rigid container. <br />Containers shall be leak resistant, have tight -fitting covers and be kept clean and in good <br />repair. <br />Immediately during the inspection Gayle Moses placed the red <br />biohazard bag in a Biohazardous waste container. <br />Continue placing all red biohazard bags in the appropriate medical waste <br />containers as specified in Chapter 9 Section 118275 and 118280 of the <br />MWMA. <br />d. Other — Narrative <br />Biohazardous Waste stored in the designated accumulation area was stored longer <br />than 7 days. At the end of the inspection this Large Quantity Generator informed this <br />Enforcement Agency that the medical waste is scheduled for pick-up every two weeks. <br />A pick-up was performed on May 6, 2011 then 2-wees later on May 19, 2011, Section <br />118280 (d) (1) (A) Chapter 9. If the person generates 20 or more pounds of biohazardous <br />waste per month, the person shall not contain or store biohazardous or sharps waste <br />above 0 degrees centigrade (32 degrees Fahrenheit) at any onsite location for more than 7 <br />days without obtaining prior written approval at the enforcement agency. <br />—+ Immediately contact with a registered medical waste hauler for <br />scheduled pick-ups every seven days. Completed 5-23-11. <br />'♦ Provide documentation showing an agreement between this Large <br />Quantity Generator and a registered medical waste hauler for pick-up to <br />occur every 7 days. On May 23, 2011 this Large Quantity Generator <br />provided an email between this Large Quantity Generator and its <br />medical waste hauler. The email shows a request by the LQG to reduce <br />scheduled pick-ups from 2 times a week to 1 time a week. Contract <br />attached. <br />
