Observations and Corrective Actions: <br />19. For steam sterilization, the temperature does not reach 250 F for 30 minutes. Thermometer is not calibrated <br />annually. <br />- Recording or indicating thermometers shall be checked during each complete cycle to ensure the attainment of 250 F <br />for at least 30 minutes. HSC118215(2)(B)-Submit evidence of the thermometers annual calibration within 30 <br />days. <br />24. Pharmaceutical waste is not segregated for storage. <br />- Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management <br />plan. Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br />- Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br />container or secondary container labeled with the words "HIGH HEAT OR INCINERATION ONLY" on the lid and <br />sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)-The following areas had pharmaceutical <br />containers without the label on top: 2 West Dirt Utility Room and SICU Dirty Utility Room. Materinity Dirty <br />Utility Room had a sharps container with pharmaceutical waste. Submit photographic evidence of correction <br />for the containers without the top label within 30 days. <br />Notes: 1. Stericycle is the medical waste hauler. <br />2. Treatment and tracking records were available for review. <br />3. Some manifests did not indicate where the medical waste was going for treatment. Facility was instructed <br />to ensure that subsequent manifests indicate where the waste is going for treatment. <br />4. Manifests were completed for the drop off of racks used to hold medical waste containers. Facility was <br />informed that manifests are not required for this situation. <br />Reinspecion onlabout: A reinspection fee of $130 per hour may be charged.
