This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br />documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, <br />and shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a) <br />Ensure subsequent tracking record shows the treatment method. Submit a copy of tracking record that shows the <br />treatment method used within 30 days. <br />24. Medical waste is not segregated at point of generation. <br />- Medical waste shall be contained separately from other waste at the point of origin. HSC 118275(a)(1) Sharps container locat( <br />west of the 2"d floor at the nurse station contained batteries. Submit photographic evidence of correction within 30 days. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on th <br />sides so as to be visible from any lateral direction. HSC 118280(c) Biohazard waste in the soil utility RACU room on the 4th floo, <br />had biohazard bags in a container that was not cover with a tight fitted lid.
