Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) — Biohazard containers were observed in the utility rooms <br />of the Respiratory ICU (4th floor), 2 South ICU (Pavilion) and the Labor/Delivery (Pavilion) without the appropriate labels <br />(see attachment; PHOTO 5, PHOTO 8 and PHOTO 9). The labels on the containers in RICU and South ICU were <br />corrected on site. Provide photographic evidence to the EHD of the appropriate labels on the container in the Labor/ <br />Delivery utility room. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. HSC 118280(c) —Biohazard containers did not have tightly fit lids in the <br />utility rooms of the Surgery Unit (2nd floor) and Labor/Delivery (Pavilion) (see attachment; PHOTO 6 and PHOTO 9). <br />Provide photographic evidence to the EHD of the Biohazard containers in the Surgery Unit and Labor/ Delivery with <br />lids. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) —The container used for biohazardous waste was <br />overflowing in the Labor/Delivery Unit (Pavilion) (see attachment; PHOTO 9). A hamper was observed in Labor/ Delivery <br />that contained biohazardous waste (see attachment; 10). Biohazardous waste should be placed in a rigid container. <br />Provide photographic evidence to the EHD of biohazardous waste contained in rigid containers in Labor/Delivery. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) — The Biohazard containers located in the utility rooms of <br />ER South and ER North were not lined (see attachment; PHOTO 11 and PHOTO 12). Line the containers with biohazard <br />bags, provide photographic evidence to the EHD. <br />31. Sterilization operator training records not available. <br />The operators of the treatment equipment shall be required to receive training in the operation of the treatment equipment, proper <br />protective equipment to wear, if any, how to clean up spills, and other information required to operate the treatment equipment in <br />a safe and effective manner. <br />- Annual training for the operators shall be provided after the initial training has been completed. HSC 117938 - Records <br />indicate the last training provided to sterilization operator(s) was held 2016. Provide training to operators on an annual <br />basis, submit documentation to the EHD. <br />Submit all corrections to the EHD on or prior to January 12, 2020. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
