OBSERVATION: A new designated operator, Eric Hinojasa, began conducting inspections on August 25, 2023 and notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator Identification Form" no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation, Class II. 203 CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. Work orders for August 2023 dispenser replacement and September 2023 secondary containment testing were not found on site. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site. Submit copies of any missing work orders and alarm records to the EHD. This is a repeat violation, Class ll. 205 CCR 2712(i) Failure to submit and have current UST Monitoring Plan readily accessible at the facility. OBSERVATION: An approved copy of the monitoring plans were not available for inspection. REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a repeat violation, Class II.
