Main Street <br /> Stockton, CA 95202-3029 <br /> Dear Alfonso, <br /> The following are the corrective actions taken as a result of the inspection completed on 5-19-11 <br /> & 5-23-11 by the Environmental Health Department for Lodi Memorial Hospital at 975 S. <br /> Fairmont Ave., Lodi, CA 95240 and Lodi Memorial Hospital West at 800 S. Lower Sacramento <br /> Rd., Lodi, CA. <br /> Lodi Memorial Hospital <br /> 1. Section 117975 —LQG shall keep records for 3 years <br /> a. A medical waste generator shall maintain medical treatment, and tracking records, if <br /> medical waste is removed from the generator site for treatment, for 3 years. <br /> Within 30 days provide review to this Enforcement Agency treatment <br /> records showing pharmaceutical waste containers removed from the <br /> generator's site received treatment as specified on Chapter 8 Section <br /> 118222 (b). The treatment records this Enforcement Agency is <br /> requesting are for Bio-Systems pick-up of pharmaceutical waste <br /> containers. <br /> Stericycle has provided us with the manifest for the Bio-Systems pick- <br /> up indicating the final incineration destination for the pharmaceutical <br /> waste front.January 08,2010 through June 17, 2011. Stericycle has <br /> added two boxes with the final destination of either Fresno, CA <br /> (transfer/treatment station for Sharps and biohazard waste) or North Salt <br /> Lake, Utah(treatment facility for incineration medical/pharmaceutical <br /> waste). The driver is responsible for marking appropriate boxes and <br /> weights associated with the waste designated for the appropriate final <br /> disposal destination. <br /> 2. Section 118275 —MW red bags,path, and chemo shall be labeled properly. <br /> a. To Containerize or store medical waste a person shall do all of the following: <br /> b. Biohazardous waste except biohazardous waste as defined in subdivision(g) of <br /> Section 117635 shall be placed in a red biohazard bag conspicuously labeled with the <br /> words "Biohazardous Waste"or with the international biohazardous symbol and the <br /> word"Biohazard". <br /> 1 <br />
