On 1/12/2023, an email was sent to the contractor documenting the missing items and errors.A revised <br /> secondary containment test report was not submitted. <br /> REGULATION GUIDANCE:A copy of the'Secondary Containment Testing Report Form'must be submitted within <br /> 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> This is a minor violation. <br /> 317 CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. <br /> OBSERVATION: The 91 line leak detector failed to detect a leak when tested. <br /> REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> CORRECTIVE ACTION: The leak detector was replaced and retested during the inspection. <br /> This is a Class II violation. <br /> 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. <br /> OBSERVATION: The 87 spill container failed when tested. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to repair or <br /> replace the spill container under permit and inspection of the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 6/14/2023. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($156).
