#115-we've received the monitoring system certification with the revision to the "All monitoring equipment is <br /> operational..." question on page 2 of 6, but a complete alarm history was not provided showing all the sensors tested <br /> 8/30/2022. Review#115 on the 12/29/2022 amended report.The attached alarm histories had missing alarms for <br /> 8/30/2022 due to multiple alarms going off on 8/31/2022. I've already previously discussed with Isaac Anderson the <br /> incomplete alarm history and he attached the closest thing he could find to a complete alarm history, a designated <br /> operator alarm history from 8/31/2022 but this still does not list all the alarms tested 8/30/2022. If you are unable to <br /> provide a full alarm history showing all sensors tested on 8/30/2022, please provide a statement addressing how <br /> complete alarm histories showing all tested sensors will be provided moving forward. <br /> #118—closed <br /> #203—the attached documents do not suffice to close the violations. Monitoring records for the past 3 years were not <br /> requested. Please review#203 in the 12/29/2022 amended inspection report. We don't have access to this eBinder that <br /> was listed in the statement. Notes below on the requested items from the 12/29/2022 amended inspection report(the <br /> two crossed out items were attached in the document provided). <br /> The FMS Site Compliance Reports are not alarm histories or work orders.These are showing tank and line tests that <br /> were performed as part of a Costco monitoring program per the statement at the bottom of the document.This is not <br /> what is needed. We need alarm histories showing alarms that have occurred. See the alarm histories attached to the <br /> Monitoring System Certification you attached for#115 as an example.The alarm histories are the pages behind the <br /> report. Work orders are like the Jones Covey work order that show a date and what work happened on that date for this <br /> site. <br /> • Alarm history print out for all alarms that occurred after June 2022 designated operator report until the <br /> monitoring panel replacement that occurred during the 91 tank installation and fueling station upgrade. <br /> The alarm history provided was for 5/1/2022 to 6/7/2022.The history needed would be after the time <br /> frame you provided.The alarm history need would be from whenever the June 2022 designated <br /> operator report was through when the panel was replaced. For reference the tanks were placed in the <br /> ground on 6/22/2022 and all of the final testing occurred 8/30/2022. <br /> • Alarm logs and response records for all alarms that occurred during the 91 tank installation and fueling <br /> station upgrade.This means responses to each alarm that occurred and that they were responded to as <br /> the tank was not temporarily closed during the renovations, so the UST should have been continued to <br /> be monitored during this whole renovation. <br /> • jenes I^.,yey Wed(erder fer 9/141/2022 fer dreg tube <br /> • Jones Covey work order for 9/27/2022 for drop tube inspection was not attached. Per Bruno's email on <br /> page 144 of your document, he stated they received a call on 9/27 and a technician inspected the drop <br /> tubes. A work order was not attached.
