I've reviewed your submitted return to compliance documents sent 12/7/2022 and the alarm history <br /> sent on 1/13/2023. <br /> The following violations have been closed: 214, 301, 302, 313, 315, 316, 318, 319, 320, 327 <br /> The following violations remain open. See amended inspection report for more information and the corresponding <br /> Return to Compliance Certification and Corrective Action Statement for the 12/29/2022 report. I've attached both to <br /> this email for reference: <br /> 111—pending a statement <br /> 115—we've received the monitoring system certification with the revision to the "All monitoring equipment is <br /> operational..." question on page 2 of 6, but a complete alarm history was not provided showing all the sensors tested <br /> 8/30/2022.The attached alarm histories had missing alarms for 8/30/2022 due to multiple alarms going off on <br /> 8/31/2022. If you are unable to provide a full alarm history showing all sensors tested on 8/30/2022, please provide a <br /> statement addressing how complete alarm histories showing all tested sensors will be provided moving forward. <br /> 118—pending a statement <br /> 203—pending the following documents listed on the 12/29/2022 amended report: <br /> • Alarm history print out for all alarms that occurred after June 2022 designated operator report until the <br /> monitoring panel replacement that occurred during the 91 tank installation and fueling station upgrade. <br /> The alarm history provided was for 5/1/2022 to 6/7/2022.The history needed would be after the time <br /> frame you provided.The alarm history need would be from whenever the June 2022 designated <br /> operator report was through when the panel was replaced. For reference the tanks were placed in the <br /> ground on 6/22/2022 and all of the final testing occurred 8/30/2022. <br /> • Alarm logs and response records for all alarms that occurred during the 91 tank installation and fueling <br /> station upgrade.This means responses to each alarm that occurred and that they were looked at and <br /> responded to as the tank was not temporarily closed during the renovations so the UST should have <br /> been continued to be monitored during this whole renovation. <br /> • Jones Covey work order for 9/14/2022 for drop tube inspection <br /> • Jones Covey work order for 9/27/2022 for drop tube inspection <br /> • Jones Covey work order for 10/10/2022 when 87A (T2) drop tube was replaced <br /> • All work orders for the tank installation and upgrades <br /> 208—pending a statement that addresses both items noted under#208 on amended report 12/29/2022 <br /> 308—pending a statement.#309 was replaced with#308 on the 12/29/2022 amended report. A statement is needed for <br /> #308. <br />
