The medical waste management plan(MWMP)does not contain all information listed in HSC 117960. <br /> -MWMP shall contain all information listed in HSC 117960. HSC 1 17960-Submit an updated copy within 30 days. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and shipping and <br /> tracking documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records, and shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC 117975(a)-Submit one complete tracking and treatment record within days. <br /> 24. Pathology waste is not segregated for storage. The secondary container is no;labeled appropriately. <br /> -Pathology waste shall be segregated for storage, and the secondary container shall be labeled with the words <br /> "Pathology Waste"or"PATH"on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(5)- <br /> Submit photographic evidence that the improperly labeled pathological waste container(was actually a <br /> garbage container)in the L&D soiled utility room was replaced with a properly labeled pathological waste <br /> container within 30 days. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> - Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or <br /> tightly lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" <br /> or BIOHAZARD". HSC 118285-Submit photographic evidence that the sharps container in the secondary <br /> container labeled "incineration only"was removed and the secondary container holds only a pharmaceutical <br /> waste container within 30 days. <br /> 30. The designated accumulation area is not secured or marked with proper signage. <br /> -A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked <br /> with warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS <br /> KEEP OUT", in English and in another language determined to be appropriate by the infectious control staff or LEA. <br /> HSC118310-Submit photographic evidence that signgage is on the outside of the designated accumulation area <br /> and is in both English and Spansih within 30 days. <br /> Notes: 1. Stericycle is the medical waste hauler for this facility. <br /> 2. There are empty medical waste containers stored outside next to the designated accumulation area. The <br /> containers are in a locked area,however one side of the area has small wall separating the area from <br /> residences. It is strongly recommended that the empty containers are put inside the designated <br /> accumulation area. <br /> Reinspecion onlabout: A reinspection fee of$130 per hour may be charged.
