A preliminary review of paperwork submitted except drawing <br />plans indicate that the following were not submitted. Please provide the additional <br />information (two copies each) . <br /> <br />1) Page 3 of the Application for Underground Storage Tank Installation Permit: <br /> indicate the Contractor’s name, phone, address, license number, hazardous waste <br />certificate, worker’s comp number, fire district permit number, Board of <br />Equalization number for underground storage tanks, tank ID numbers, and tank <br />sizes per grade stored. <br />2) Page 6, Section F (primary and secondary) of the permit application: <br />Provide accurate piping model numbers for all piping being used, including the <br />use of multiple sizes of vapor piping (if applicable). <br />3) Page 7, Section G of the permit application: <br />Identify which overfill prevention method will be utilized at this facility. If a <br />mechanical flapper will be utilized, drop tube measurement calculations will need <br />to be provided prior to install. Section H identify the piping and tank corrosion <br />protection. <br />4) No training certifications were provided. Provide current contractor’s training <br />certifications for installation and testing of all proposed UST <br />equipment/components, including but not limited to: tanks, sumps, sensors, <br />monitoring panel, leak detectors, piping, spill containment, and penetrations. <br />5) Provide current ICC UST service technician and UST installer certifications for <br />the individual performing the work.
