A preliminary review of paperwork submitted except drawing <br />plans indicate that the following were not been received. Please provide the <br />additional information . <br /> <br />1) Page 3 of the Application for Underground Storage Tank Installation Permit: <br /> indicate the Contractor’s name, phone, address, license number, hazardous waste <br />certificate, worker’s comp number, fire district permit number, Board of <br />Equalization number for underground storage tanks. <br />2) No training certifications were provided. Provide current contractor’s training <br />certifications for installation and testing of all proposed UST <br />equipment/components, including but not limited to: tanks, sumps, sensors, <br />monitoring panel, leak detectors, piping, spill containment, and penetrations. <br />3) Provide current ICC UST service technician and UST installer certifications for <br />the individual performing the work. <br />4) Submit a work plan for enhanced leak detection (ELD) testing, to be conducted by <br />a properly licensed individual. <br />========== <br />5) Provide manufacturer’s cut sheets for all proposed UST system component. <br />Received: sensors, leak detectors, turbine (STP) sumps, fill sumps, under <br />dispenser containment sumps (UDCs), penetrations, and spill containers - for <br />detail review with drawings. <br />6) Provide buoyancy calculations signed by a Professional Engineer which <br />demonstrate a safety factor greater than 1.25.- for detail review with drawings.
