The <br /> testing was incorrectly documented as 12/6/2023. The correct date for the monitoring system certification and spill <br /> container testing is 12/6/22. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: The overfill prevention equipment(OPE)testing was last performed on 11/15/2022. The 87 OPE <br /> failed on 11/15/2022. It was replaced and retested on 02/24/2022.At the time of the inspection,the technician was <br /> unable to verify whether or not any flow restrictors were installed that could interfere with overfill prevention <br /> equipment operation.The 02/24/2022 test is incomplete and the 87 tank's compliance with OPE requirements has <br /> not been verified. <br /> REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October <br /> 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> CORRECTIVE ACTION: Immediately schedule the completion of this test and provide 48 hours notification to the <br /> EHD. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by January 25, 2023. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($162).
