resolved. I've attached both the 2021 and 2022 inspection reports for reference. To close #214 out, you
need to apply for a permit for the leak detector that was replaced on 5/21/2021 and we need to witness the
testing. Whenever monitoring parts are replaced on the UST system you need to notify EHD. Some items
don't need permits, for example if they are like-for-like,but notifications always need to be sent to EHD.
This leak detector that was replaced on 5/21/2021 did not have any notifications to EHD which causes the
double fee penalty and the need to pull a permit for work that has already been conducted. Please read both
inspection reports for information as well as the UST Service Request Guidance which has information on
permits; I attached all to this email. See link for UST permit applications
(https://www.sjgov.orWdepartment/envhealth/forms v.orWdepartment/envhealth/forms ) you need to complete forms "UST Repair or
Retrofit" and"UST Service Request" and submit to UST(d),sjgov.org .
If you have any questions please let me know.
All the Best,
Carol Presto
Registered Environmental Health Specialist
San Joaquin County Environmental Health Department
Email: cprestoksjgov.org I Direct: (209) 616-3061 Main: (209)468-3420
https://www.sjo vorg/department/envhealth/
*NOTE*I will be out of office every other Friday:
January 6,20 1 February 3, 17 1 March 10,24 1 April 7,21
IIIII
From: Abed Alajarmeh<abedAcasyner y com>
Sent: Tuesday, January 17, 2023 10:13 AM
To: Marathon Tracy<marathontracy950(d),gmail.com>; Presto, Carol [EHD] <cpresto(d sjgov.org>
Cc: Sara Jarrett<sara@casynerg_y com>
Subject: Re: 950 W Eleventh St-RTC and Operator Question
Dear Carol
12
