HSC117960—Review and revise MWMP as needed <br /> and submit by 1/2/2015. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> - A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)— Facility was missing proper treatment and tracking records for pickups done by Stericycle in <br /> January and February. From March 2015 forward, facility has proper treatment and tracking records for <br /> pickups done by Stericycle and Daniels Sharpsmart Inc. No corrective action needed. <br /> 24. Pharmaceutical waste container located in the procedure room was not labeled properly. <br /> -Nonradioactive and non-RCRA pharmaceutical wastes,that are regulated as medical waste, shall be placed in a <br /> container or secondary container labeled with the words"HIGH HEAT" OR"INCINERATION ONLY"on the lid and <br /> sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)—Pharmaceutical waste containers stored <br /> in interim storage area in the Surgery building were missing labels on the lid. Add labels and submit evidence <br /> of compliance by 1/2/2015. <br /> Notes: <br /> 1.) Facility uses Daniels Sharpsmart Inc. for removal of pharmaceutical and biohazardous sharps waste. <br /> Biohazardous sharps and pharmaceutical waste is consolidated in pharmaceutical waste containers which are <br /> picked up weekly by Daniels Sharpsmart for incineration. <br /> 2.) Facility uses Stericycle for the removal of biohazardous waste. <br /> 3.) Facility's small biohazardous waste bags meet ASTM D1709. All of the small biohazardous waste bags are <br /> placed into large biohazardous waste bags which meet both ASTM D 1709 and ASTM D 1922 before being <br /> stored in the designated accumulation area for pickup by Stericycle. <br /> Reinspecion on/about: A reinspection fee of$130 per hour may be charged.
